New Patients

What should I bring when I come to enTrust Immediate Care for treatment or services?Show Answer

You must bring valid photo identification, which can be a driver's license, state-issued ID card, passport, military ID, employment badge, or school ID card.  Your photo identification will be copied and kept on file.

  • If you have insurance, you must also bring your current insurance card.  Your proof of insurance will be copied and kept on file.
  • If the patient is a minor under 18 years of age and not accompanied by a parent or legal guardian, the accompanying adult must have a notarized statement signed by the parent/guardian authorizing treatment. The parent/guardian/ adult will be asked for photo identification, current insurance, and (as required) proof of guardianship.  This information will be copied and kept in the minor's file.
Are there special procedures for minors?Show Answer

A parent or legal guardian should accompany any minor under the age of 18.  If you are not the parent or legal guardian, you will need a notarized statement signed by a parent or legal guardian authorizing you to give permission for services and billing, if appropriate.  Proper identification is also required (see FAQ above for information on required identification and information related to the treatment of minors). 

How can I obtain a copy of my medical records from enTrust?Show Answer

You may request a copy of your medical record from enTrust. You may contact Horizon Billing Specialists at 1-800-378-9991. With proper identification provided, they will provide the records you request via email, fax or US Mail. 

Follow Up Care

What if I need follow-up care after I visit enTrust?Show Answer

Should you require follow-up care after seeking treatment at enTrust, we will provide you with a copy of all the necessary documentation and reports for you to share with your primary care physician.  If you don't have a primary care physician, follow-up treatment can be provided at enTrust. 

Infants and Toddlers

Does enTrust treat infants and toddlers? Show Answer

Yes, as long as the infant is at least 6 months of age.  For infants under 6 months, we recommend seeing a pediatric physician or visiting your local hospital emergency room.

Wellness and Routine Exams

Can I come to enTrust for routine exams/illnesses, or is the facility only available for emergency/non-life-threatening medical issues?Show Answer

At enTrust Immediate Care, we are available to patients who need illness/injury-related care as well as patients who require general health and wellness services. Our state-of-the-art facility, along with our on-site physicians, nurse practitioners, and medical technicians, enables us to provide not only Urgent Care, but also Family Care & Wellness, Travel Medicine, Sports Medicine, and Employer Services/Occupational Medicine. We are equipped see infants, children, and adults for routine physical exams, wellness exams, diagnostic and laboratory testing, X-rays, EKGs, and much more.
